To start out, I perform a capability audit where I end up diagnosing both evident and unseen opportunities for improvement and identifying these in a "Findings" report. This is followed by creating a no-risk environment within management for the discussion of the tough issues that are squelching profits.
Then it becomes time to expand the responsibility for profit generation and cost reduction to more than a few people in the organization and get everyone to focus on the welfare of the company as a whole instead of on individual departments.
Activities are continually tracked and monitored and PERFORMANCE METRICS are established.
